Home & Health Resources (H&HR) has been providing private home care services on the South Shore since 1983.  Along with South Shore Visiting Nurse Association (SSVNA) and Hospice of the South Shore, H&HR merged with South Shore Hospital in 1994 to become the Home Care Division of South Shore Hospital.  H&HR provides highly skilled private duty nursing, personal care and household management services to residents of 30 towns and cities in Norfolk and Plymouth counties.

H&HR has been recognized for service excellence through Accreditation by the Home Care Alliance of Massachusetts.  To achieve this honor, the H&HR Team has demonstrated competencies of skill and care by providing evidence that quality of care is our number one goal when working with our clients and families at home and in the community.

Services are available and reimbursed through several funding sources which include:  self pay, Elder Services authorization, long term care insurance, worker’s compensation, veteran’s administration, private insurances and Medicaid for private duty nursing.

In addition to H&HR core services that meet home care needs of South Shore clients, several other programs are available through H&HR.  Through South Shore Elder Services, specialized services are offered to consumers who benefit from skilled nursing, Habilitation Therapy, Smoking Cessation and Incontinence Programs.

H&HR provides educational and health promotion activities throughout the South Shore.  SSES has awarded grant funding for an educational and supportive series entitled “My Life My Health,” which focuses on self-management of chronic disease conditions.  Through private grant funding, the very successful “First Day Home Program” has been made possible.  Frail elders receive a nursing visit at home on the day of discharge from hospital, skilled nursing facility or rehabilitation facility.  The program has proven most beneficial in easing the transition home and preventing re-hospitalization through medication management, home safety, caregiver teaching and referral activities. The Alzheimer Association Habilitation Therapy Program is also grant funded, with a focus on education and support to caregivers of Alzheimer and Dementia clients.
